Routine clinical decision is based on personal knowledge of pathophysiology and clinical experience. Our understanding of the pathophysiology of skin diseases is evolving constantly as a result of explosive growth in basic research in dermatology. Decisions based on “pathophysiologic reasoning” could, therefore, change quickly. Likewise, the biological plausibility of several therapeutic measurements used in dermatology can eventually been rejected by emerging knowledge, or worse, certain interventions may be shown to be dangerous after clinical trials or overall assessment of previous trials. Moreover,
the volume of dermatological literature is
growing exponentially and there is no direct relation between the quantity and quality of published information. Therefore, a hierarchy of evidence has to be established based on study design and reflecting the relationship between quality and the likelihood of bias.


Evidence-based medicine is defined as “the conscientious, explicit and judicious use of current best evidence” when making clinical decisions about the care of individual patients. Evidence-based Dermatology is the application of the principles of evidence-based medicine to the care of patients with skin diseases. The aim of Evidence-based Dermatology is to support the clinical decision by providing a system to locate, appraise and apply the best available information in order to improve the outcome of patients with skin diseases. The process includes four major steps, namely asking a clinical question, finding the evidence, critically appraising the evidence and integrating the new information into clinical expertise and judgment.


On the other hand, the tools of Evidence-based Dermatology allow a better prospective plan of clinical diagnostic and therapeutic studies, which can reflect a high level of quality and offer valuable and unbiased information for clinical decisions.
